Shri Khirud Baruah of Bagaljan G.P, Raha, Nagaon, alone entered 1405 numbers of member record (210 forms) on 12th of November. He took 12 hours at a go to achieve this feat. On 6th Nov, 2015 Operator Shri Dibya Jyoti Das of Amtola NSK, Raha Circle, Nagaon alone entered 100 forms (500 member records) in DOCSMEN software. On 6th Nov, 2015 Operator Shri Sanjay Das of Amtola NSK, Raha Circle, Nagaon alone entered 100 forms (500 member records) in DOCSMEN software. On 10th Nov, 2015, Shri Priyangshu Arandhara of Gharpholia NSK under Mariani Revenue Circle of Jorhat alone entered 133 forms and record of 532 members. On 13th November, Operator Shri Mrinal Kanti Deka, under Nalbari Revenue Circle, Nalbari alone entered 100 forms in DOCSMEN (Document Segregation and Meta Data Entry) -- a software designed to electronically send documents to the issuing authority for Verification.
Ms Daisy Thakuria of Hatigorh SIDEC center (morning shift) on 24th Nov, 2015 takes the top spot by entering 547 member record through the NEWGEN Application. Ms Dipti Rani Gogoi (L) and Ms Damini Brahma of SIDEC Hatigorh center each entered 469 member record thereby becoming the top performers of 24th Nov. Ms Leena Thakuria of Chandamri SIDEC Center (morning shift) last week alone entered over 600 member record daily thereby taking the top performer spot. Ms Shyamolima Sarma of Ganeshguri SIDEC center alone entered 467 member record thereby occupying the top slot on 23rd Nov. On 12th Nov, 2015, Dhrubo Jyoti Das of Hatigorh SIDEC center, evening shift, alone entered 463 numbers of member record.
On 12th Nov, 2015, Sajida Begum of Ganeshguri SIDEC center alone entered 455 numbers of member record. On 28th November, Shri Nabajit Nath of Balijana 9 NSK under Balijana Circle of Goalpara alone entered 200 Forms (734 member records). Shri Bendanta Lahkar entered 811 member record on 20th Nov thereby becoming the top performer for NEWGEN Application - SIDEC Chandmari. Shri Jayanta Chauhan of SIDEC Chandmari centers working at night shift, alone entered 812 member record on 23rd Nov and takes the top performer spot for NEWGEN Application. Shri Nayan Mani Baishya of Hatigorh SIDEC center alone entered 614 member record through e-Form Application on 20th Nov, taking the second spot.
Operators of West Circle, Jorhat are working till (12.30 AM) at the Circle hub , DRDA complex on 16th-17th Dec, 2015. Shri Dhruba Jyoti Das of Khakarpur GP of Bongaigaon Circle entered 199 forms consisting of 487 member details into the DOCSMEN software on 14th Dec, 2015. Shri Jayanta Borgohain of Gakhikhua NSK under Teok Circle from Jorhat alone entered 251 forms in DOCSMEN on 14th December, 2015. Shri Jitu Rajput of Milon GP NSK, Mahmora Rev. Circle Sivasagar on 17 Dec 2015 entered 261 forms and members 1165 through DOCSMEN software. Shri Lakhyajyoti Saikia(left) entered 130 forms in DOCSMEN software on 14th Dec, 2015 under Titabar Circle, Jorhat.
Shri Nikunja Das, Khagarpur NSK, Bongaigaon felicitated for entry of 360 forms consisting of 1369 members in e-form software on 14th Dec, 2015.
 
 
On 17th August, 2015, the NSK Team of Choutara Community Hall under Goroimari Revenue Circle of Kamrup received the highest number of Application Forms (1284) until date. The team was headed by Circle Officer Shri Pradip Bhorali, A.E, P H E, Boko sub division and LRCR Shri Asif Hussain Mazumder, JE,PHE,Boko Sub division. NSK Team of Bisondoi G.P office in Dhubri district completed receipt of Application Form (2582) on 13th Aug’15. Headed by LRCR Officer Shri Ashok Kr Srivastava (ADO Barundanga), Shri Anarul Haque (ALO) the Star Performers of the team are Smt Rashmi Rekha Roy and Shri Dwipok Roy. On 13th Aug’15, NSK 4 Mayong (i) of Chandrapur Revenue Circle under Kamrup (M) completed the receipt of Application Forms with a total of 1888 forms received. Supported by Circle officer, Lanka and LRCR Officer Shri Zahidul Islam, Extension Officer, the NSK Team of Laskar Pathar GP, Lanka Circle of Nagaon district created a new record by processing 1215 Application Forms on 8th, August, 2015 On 11th Aug’15 NSK Kokrajhar C (Timber Merchant) NSK, Kokrajhar Circle, Kokrajhar district received 1795 Application Forms thereby completing 100% receipt of forms. Guided by LRCR Shri Shayam Sundar the Star Performers are Shri Mithinga Baglary, Shri Pulak Kumarr Nath (LO), Shri Deepak Kr Kalita (LO), Shri M Mushahary (LO) and Smt Rina Das.
On 10th August, 2015, Smt Trishna Kalita of NSK Kandhulimari GP West of Nagaon district created an individual record in the state by becoming the First NSK Operator who has alone received 300 Application Forms in a single day. On 19th August NSK 18 of Katigorah Circle in Cachar processed 1600 Application Forms which is the state highest so far. On 18th August Team of NSK Golakganj 1 under Golakganj Circle in Dhubri district received 1415 numbers of Application Forms On 18th August Golakganj 1 NSK Team under Golakganj Circle in Dhubri district received 1415 numbers of Application Forms On 19th August, NSK Team of Office of CDPO Katigorah Cachar processed the highest number of forms in a day thereby taking the count to 1600.
On 23rd August 2015 Star Performer Rajesh Nath of Katigorah Circle, NSK 19 of Cachar processed and uploaded 510 Application Forms, which is the highest solo record until 24th August, 2015. On 21st August, 2015, NSK Bishnupur Gaon Panchayat office under Dhemaji Revenue Circle of Dhemaji district have successfully completed 100 per cent receipt of Application Forms.
